What’s on the ticket
The delivery ticket is the paper record of one truckload of ready-mix concrete leaving the plant and reaching a customer. This template covers the fields most small producers actually use:
- Customer + job — customer name, phone, job site address or name, PO / job number, ordered yardage.
- Load and mix — mix design ID, load size (yards), target slump, target air percent, additives.
- Seven timestamps — batched, left plant, arrived on site, began pour, completed pour, left site, returned to plant. The backbone of both delivery pacing and any dispute over on-site time.
- Water log — plant water added, site water added, with a customer-authorization initials column for site water. Protects the producer if strength results come back low after site water was added at the customer’s request.
- Charges — yards delivered, unit price, subtotal, waiting-time / stand-by charges, fuel surcharge, tax, total.
- Signatures — driver, customer receiver.
PDF vs. Excel
The PDF is set up for print-and-write — the classic carbon-copy ticket layout, ready for the driver to fill by hand in the cab. Fields are sized for a clipboard, not a browser window.
The Excel version has the same fields plus auto-calculating totals (subtotal, waiting-time, tax, grand total) and can be typed on a phone or tablet. Handy for offices that email tickets instead of collecting paper.
