Mixer Truck Pre-Trip Inspection

A general pre-trip inspection form for concrete mixer trucks — 35 line items across six sections, with a defect log and a driver condition certification. Free download in both PDF and Excel.

What’s on the form

A pre-trip inspection is what the driver does before rolling out of the yard: a walk-around and cab-over that confirms the truck and its mixer are in condition to run the day’s pours. This template covers 35 check-points grouped into six sections, plus two closing blocks:

  • Header — truck number, driver, odometer, date, time, weather.
  • Six inspection sections covering the engine bay and fluids, cab and instrumentation, brakes and steering, tires and wheels, lights and signals, and the mixer-specific components (drum, chute assembly, water tank + fittings, hydraulic controls, seals and bearings). Each line item gets a pass / attention-needed check.
  • Defect log — space to describe anything that didn’t pass, so the shop or supervisor knows what to look at before the truck is dispatched.
  • Driver condition certification — signature and time stamp confirming the driver has inspected the equipment and reported any defects found.

PDF vs. Excel

The PDF is set up for print-and-write in the cab — big check boxes, room for a pen. The typical use: a stack in the yard, filled and turned in each morning.

The Excel version has the same fields for teams that prefer to keep the record on a tablet or phone. Signatures still need to be captured somehow (touchscreen scribble, initials, an attached photo) — the template gives you the fields, not the workflow.

A note on compliance

This is a general pre-trip inspection form we built for our own use and are sharing free. Whether it satisfies any specific jurisdiction’s pre-trip requirements is your call — regulations vary by state, agency, and vehicle class, and none of the fields here have been reviewed against any regulatory checklist. If your operation has a required form, use that one; this template can still be useful as a supplemental walk-around checklist.
