What’s on the sheet
A mix design record is the producer’s written recipe for a specific concrete mix — what the mix is targeting, what goes into it per cubic yard, and what came out when it was trial-batched. This template covers the fields most small producers actually record:
- Design criteria — target compressive strength (f’c and test age), slump, air content, water/cement ratio, and intended exposure class.
- Batch weights per yd³ — one row per material (cement, supplementary cementitious materials, coarse aggregate, fine aggregate, water), with source, specific gravity, and saturated-surface-dry weight.
- Admixtures — product, manufacturer, and dosage per hundred pounds of cementitious material.
- Trial batch / test results — measured slump, air, temperature, unit weight, and compressive strength at test age.
- Approval block — QC signature, date, and mix ID for cross-referencing back to batch records.
PDF vs. Excel
The PDF is set up for print-and-write — one page, ready for a clipboard next to the trial batch.
The Excel version has the same fields plus an absolute-volume calculation per material (weight ÷ specific gravity ÷ 62.4) and a running total. The sheet flags whether the total hits the 27.000 ft³/yd³ yield target so you catch a mix that’s over- or under-yielded before it ever leaves the plant.
A note on scope
This is a general mix-design record we built for our own use and are sharing free — it doesn’t certify the mix against ACI 318, ASTM C94, or any other standard. If your job requires a mix design signed off by a licensed engineer or that meets a specific agency’s requirements, this template is a useful working format but not the submission document itself.
